One hundred ninety four arthroscopic Bankart repairs were performed on a consecutive series of patients with traumatic anterior shoulder instability by the senior two authors. There were twenty-one patients with recurrent dislocations in the one hundred ninety four patients who had arthroscopic Bankart repair performed, an eleven percent recurrence rate. Fourteen of the twenty-one recurrent dislocations had significant bony defects, including three “engaging” Hill-Sachs lesions and eleven “inverted pear” Bankart lesions. The recurrence rate for patients without significant bone defects (4 percent) was found to be significantly lower than the recurrence rate for patients with significant bone defects (67 percent).
