A small number of patients have pain that is difficult to control using conventional methods. Intrathecal drug delivery (ITDD) may be useful in selected cases. This paper outlines the differences between intrathecal and epidural drug administration from both external and internal systems. Understanding some of the concepts involved in spinal drug delivery allows better selection of patients and choice of spinal drugs. Case histories of ten patients implanted with ITDD systems over a five-year period are used to illustrate the points made.
