Rupture of the posterior tibialis tendon is uncommon. Given its subtle physical signs, the diagnosis can often be overlooked and often missed. Posterior tibialis tendon disorders present in a variety of ways, ranging from intrasubstance tears through partial to full ruptures. This report highlights our present understanding of posterior tibialis tendon pathology and possible future areas of study, together with hypotheses that could explain the causes of ruptures.
