Description
As a multi-dimensional map of the expected process of
care, a patient ICP is a very useful tool in quality and performance
management within a health care system. It brings together
clinical governance, evidence based practice, audit, policies,
protocols, guidelines and multidisciplinary notes at the level
of the individual patient. This paper describes the practical
experience of a consultant psychiatrist in designing, implementing
and evaluating integrated clinical pathways at primary care
trust level.
