Clinical Risk Management is the process by which clinicians can make a significant impact on improving the outcomes of their patients. Adverse incident reporting and investigation is the backbone of Clinical Risk Management. If we as clinicians are to have the welfare of our patients at heart we must be in a position to learn lessons where things have gone wrong and put measures in place to prevent a recurrence wherever possible.
