Steroid Responsive Hepatocellular Reaction Triggered by Lansoprazole

We describe a case of a 58 year old woman who developed jaundice eight days after commencing lansoprazole for straight forward gastro-oesophageal reflux disease. Liver biopsy confirmed a severe autoimmune type hepatitis. Her autoantibodies were negative and other causes of a hepatitis were excluded by further investigation. She was treated with steroids and responded clinically and biochemically. Eighteen months after stopping steroids her liver function tests remain normal and she is clinically well. On review of the literature we can find only one recent report of hepatitis after exposure to lansoprazole. We propose that the most likely cause of her hepatitis was an idiosyncratic hepatocellular reaction to lansoprazole
