A 79 year old lady presented with biliary symptoms, and was found to have multiple common bile duct calculi. What was initially thought to be ascites, subsequently was shown to be a large subcapsular collection of bile – a biloma. This most probably formed as a result of decompression of the dilated biliary tree into the subcapsular space.
