An 81 year old lady was referred from the Emergency Department to the medical admissions team late in the evening with dysphagia. There had been 3 episodes that day when she had choked on her food and temporarily become cyanosed. The patient herself denied all symptoms, in particular any abdominal symptoms, and was adamant that she wanted to get home. Previously, she had seen a speech and language therapist about her swallowing difficulties. The recommendation had been for admission to insert a PEG tube, which the patient had refused.
