It is encouraging to be the recipient of what has become a steady flow of interesting case reports. As well as applying more stringent acceptance criteria it has also proved possible to allow a degree of “theming” for individual issues of the Journal. This may not always be possible but perhaps concentrates thought in a productive fashion. Accordingly the present issue carries three case reports, each of which revolves around an unusual and unexpected perforation. It is pleasing also that assertions from presenters are now being challenged thus establishing a two-way correspondence.

Our commissioned authors continue to provide state-of-the-art summaries of emerging areas in gastroenterology at what I believe to be a very high level. Inevitably however we will appear to omit topics that our readers consider of great importance. The editorial board is always on the look-out for new suggestions and will be pleased to consider proposals of topics and/or authors. We aim to provide a broad range of stimuli and will happily move away from the routine if a stimulating suggestion is made.

Few CME MCQ answer sheets are currently returned to the Secretariat. We are content with this so long as the questions are useful and the reason for non-return simply being the other available options for obtaining certifiable external CME points. If however there are comments on the quality or value of the MCQs these would be most gratefully received.
