Description
Primary hyperparathyroidism is prevalent amongst older people. The definitive management is surgery, which can often now be achieved reasonably safely by minimally invasive techniques following accurate localisation of the parathyroid adenoma. The condition is inevitably progressive in about a third of individuals; hence surgery should be considered in most patients with the diagnosis. In asymptomatic individuals in whom surgery is inappropriate or declined, a watch and wait approach is reasonable. Otherwise, various medical therapies can be tried, but they all have their limitations and drawbacks.
