Deaths in potential or actual recipients of organ transplants are typically multifactorial. These factors can be classified as preoperative, intraoperative and postoperative, and issues relating to both donor and recipient can be important. Autopsies in such cases often occur outside specialist transplant centres, and this article is an attempt to outline a classification to remind histopathologists of the kinds of areas that might be relevant.
