Nausea and vomiting are distressing symptoms commonly experienced by patients with advanced disease. The key to managing nausea and vomiting is to determine the cause by detailed symptom evaluation. This allows reversible causes to be treated. Furthermore, knowledge of the cause, combined with an understanding of the neuropharmacology of the emetic pathways, provides a rational, mechanistic approach to antiemetic selection. This review provides practical guidance to support clinicians. Successful management of these symptoms provides enormous potential to improve patients’ quality of life.
