Description

A 31 years old postman, previously fit and well, presented to casualty with acute ischaemia of the right leg. He gave a three day history of a painful right foot, which turned blue and felt ice cold. He had experienced intermittent twinges in the right calf during the last eight weeks. His past history was unremarkable apart from gastritis and macrocytic anaemia. He drank about two bottles of whiskey per week and smoked 20-25 cigarettes per day. He was not on any medication at presentation. His family history revealed his maternal grandfather had myocardial infarction at the age of 50.
