A 25 year old unmarried Asian girl was referred to the gynaecology clinic with primary amenorrhoea, androgenic alopecia, clitoral hypertrophy, androgen dependent hair distribution and complete closure of the vagina. On examination she was slim and had no breast development. There was no acanthosis nigricans. Her initial endocrine investigations were as follows:
