Swallowing and Dysphagia in Stroke

Swallowing is initiated and modulated by multiple areas in the cerebral cortex, and generated by the brainstem in the presence of afferent stimulation from the oral cavity. Dysphagia is common after stroke and may lead to aspiration, malnutrition and dehydration. Determining the risk of aspiration is crucial. The main modalities for assessment are bedside swallow assessment, pulse oximetry, videofluoroscopy and fiberoptic endoscopic evaluation of swallowing. Dysphagia after stroke spontaneously recovers in the majority of patients. The key principles of management are meeting the nutritional requirements, prevention and treatment of aspiration-related complications.
