At a time when there is much concern about the capacity and quality of longterm care for the most dependant and frail of our patients, I believe you will find the review by Drs Whitehead and Hendra informative and thought provoking. The published evidence underpinning good practice in this sector of geriatric medicine is too frequently sidelined so it is helpful to have it mustered and presented in such a clear manner.

In response to suggestions made by readers and colleagues I commissioned the articles on the management of non-Parkinsons movement disorders and the control of nausea by Drs Pearce and Pugh respectively, both of whom have special expertise in those fields. I am more than happy to respond to the opinion of the readership in this way, so please feel free to let me have your ideas for future editions.

I am now receiving a steady inflow of unsolicited pieces, many of which I am able to use. Short items in the form of case reports, quizzes and photo-spotters are particularly welcome, though I am also pleased to be sent full length reviews for consideration. This edition completes Volume 3; I hope you enjoy it. My plan for Volume 4 is centred around consolidating the improvement in the quality of the reviews and presentation, and striking a balance between the needs of the CME curriculum and areas which are timely and topical. I look forward to hearing your views.
