A 26 year old man presents as an emergency for open reduction and internal fixation of a fractured ankle, caused by a fall from a mountain bike during a competition race the previous night. He appears well, but an ecg is performed since he gives a history of palpitations (FigECG1). The orthopaedic house officer brings it to you as he is concerned that it is not normal.
