Abstract
The medicine of long term care is poorly researched and understood, with clinical services focusing on the assessment of needs for care rather than the process and outcome of care itself. Prevention and rehabilitation are neglected with negative consequences on morbidity and cost. A number of key components of good quality care home medicine can be described. The needs of the frailest elderly may be best served by a single cohesive organization.
