The Scanner: An Airy Foot

Description

A 83 year old man with chronic rheumatoid arthritis and renal impairment presented with a few weeks history of general malaise and inability to walk. He was clinically pale, unwell, hypotensive and in atrial fibrillation. On examination he had a right forefoot ulcer and a sinus discharging pus with marked crepitus on palpation over the dorsum of his foot.
