The vast majority of mesenchymal lesions of the uterus are smooth muscle tumours and almost all of these are benign. Diagnostic problems are encountered when these lesions exhibit unusual histological features and mimic leiomyosarcomas or endometrial stromal tumours. This short review will predominantly be devoted to smooth muscle neoplasms, the most important member of the mesenchymal tumours.
