A 76 year old man presents on your orthopaedic list for total hip replacement. He is mildly short of breath at rest, has an exercise tolerance of 20 yards on the f lat and has nebulised salbutamol at home. He is an ex coal miner and says he has had chest problems for 30 years. You find 3 CXRs (Figures 1-3). Describe these CXRs, what views are Figures 2 and 3 and why have these been taken? What do you think the diagnosis might be? What would his spirometry look like? Can his chest be improved? Outline your choices for anaesthesia.
