An 82 year old lady presents for total hip replacement. She is generally fit for her age and only gives a history of central chest pain for many years, which often comes on in bed and lasts anything from a few minutes to half an hour. She has a GTN spray for this pain which normally relieves it within 10 minutes. Her CXR (Figure 1) and ECG (Figure 2) are shown. Describe this CXR and ecg. What is the likely cause of her pain? How will this affect your anaesthetic technique?
