Non-steroidal anti-inflammatory drugs (NSAIDs) are widely used, particularly by older people. This age group is the very one that will have the least homeostatic leeway to overcome any adverse effects of the drugs. The selective COX-2 inhibitors had been widely promoted as new safer NSAIDs, but recently they have been clearly demonstrated to have a serious pro-thrombotic side effect profile. By understanding the mechanisms of NSAID side effects, it is possible to minimise them effectively so that this useful group of drugs can continue to be employed where there is a proper indication.
