Rather than resurrect the argument about whether radical prostatectomy is worthwhile or not, I propose to preach to the converted about the benefits of the perineal approach in patients selected for radical prostatectomy.

For the properly positioned patient, the perineum represents the shortest route to the prostate and I and others 1 believe perineal access conveys considerable advantages over the retropubic method including better continence, lower morbidity, early ambulation, a much lower transfusion rate and a lower anastomotic stricture rate.
