Case

A fifty-three year old plasterer was seen in the chest clinic with a history of lethargy, blocked nose, post-nasal drip, snoring, sweating and increase in size of his hands and feet. He was noted that he had coarse facial features but otherwise examination was unremarkable. He was therefore referred to further investigation.
