This article reviews current arrangements for care in the community for patients with severe and enduring mental illness. The emphasis of the governments policy has shifted from deinstitutionalisation to focusing on the public protection in care in the community. At present a variety of services exist. A review of the evidence available for two specific models of community care – case management and assertive community treatment – is followed by discussion of the difficulties with current studies as well as issues for the future.
