A pelvic surprise

Description

A 91 year woman with a past medical history of Parkinson’s disease became acutely confused and drowsy one week after commencing an opiate analgesic for chronic shoulder pain.

Apart from the signs of Parkinson’s disease, examination revealed a large smooth mass in her right iliac fossa; this had not been noted before. Her drowsiness responded to withdrawal of the opiate analgesic. A CT scan of the abdomen and pelvis was arranged to assess the mass.
