Acute coronary syndromes (ACS) remain the main cause of death in the western world. Prompt diagnosis is important, so that treatments which have been demonstrated to improve outcome, can be initiated as early as possible. The 12 lead surface electrocardiogram (ECG) is one of the first diagnostic tools used in the emergency department to assess patients with a suspected ACS. In this article we discuss the interpretation of the ECG, and the use of continuous ST segment monitoring, in the management of ACS.
