Description
Continuous veno-venous haemofiltration (CVVH) is probably the commonest method of renal replacement therapy in UK Intensive Care Units. The available information on antibiotic dosing in these patients is however limited. The main factors affecting antibiotic dosing for patients requiring renal replacement therapy are: the route of drug excretion, the degree of protein binding of the antibiotic, the type of haemofiltration system being used, the patient’s renal function and the availability of an antibiotic assay service. The article reviews the available information on antibiotic dosing in patients receiving renal support with haemofiltration systems and recommendations for antibiotic dosing are made on the basis of available pharmacokinetic data and extrapolations from data on system/filter, drug and patient factors.
