Why should we consider Prophylactic Cranial Irradiation at Diagnosis of HER-2 Positive Metastatic Breast Cancer?

There are reports of an increased central nervous system
(CNS) relapse in metastatic HER-2 over expressing breast
cancer. We have retrospectively examined a series of tumours
from breast cancer patients who received palliative whole
brain radiotherapy for the presence of HER-2 and ER
receptors. The rate of over-expression of HER-2, 57%, was
much higher than in the general population of patients with
breast cancer and the rate of ER postivity, 38%, was much
lower than expected. Our results suggest that predilection for
the CNS may be an aspect of the biology of HER-2 over
expressing breast cancer.
